While COVID-19 continues to present a substantial increased risk of mortality among older adults and individuals with underlying medical conditions in the State of Colorado, isolation of individuals in nursing and rehabilitation facilities, as well as other congregate settings impose substantial physical and mental health consequences for these patients. Social distancing and use of electronic means of communication remain strongly recommended; however, the State of Colorado, in an effort to promote the health and well-being of Colorado’s patients, will allow for indoor visitation in facilities.
The Center at Centennial continues to follow all infection control guidelines provided by the state, local and federal government.
Patients may have visitors as long as the patient is not positive for COVID-19, not showing signs/symptoms of COVID-19, and no longer in 14 day quarantine. Patients that are fully vaccinated may also have visitors in our facility.
If you would like to visit a patient that resides in our facility, please contact the Activities Director at 719-685-8888. Please note, patients may have up to two visitors at a time for a maximum of 45 minutes.
If your loved one has been fully vaccinated, visits may be allowed in patient’s room. We ask that you and your loved one keeps their masks in place at all times.
You must have a mask that covers your nose and mouth on at all times while in the facility. When you arrive, your temperature will be taken and staff will ask the following questions:
Have you experienced any of the following symptoms in the past 14 days?
Temperature of 100.0 or greater, cough, shortness of breath or difficulty breathing, chills, muscle aches or pain, sore throat, loss of taste or smell, sudden feeling of being cold with shivers and fever, headache, diarrhea, congestion, runny nose, nausea or vomiting, or fatigue.
Have you been exposed to someone with COVID-19 in the past 14 days?
If you answer “yes” to any of the above questions, you will be asked to leave the facility immediately.
You will be asked to preform hand hygiene and escorted to the designated visitation area or to patient’s room (vaccination).
During your visit, we ask that you maintain 6 feet social distancing at all times. When your visit is complete a staff member will be happy to escort you to the exit.
If at any time the facility goes into outbreak status, indoor visitations will be cancelled.
